The Admin Team have created a Virtual Reception that contains all the documents that students would normally be able to pick up from our reception in Rathbone.

How can I get a PLSP?
You need to take a number of steps:
- Register with Disability Services. You can do so at https://apps.bangor.ac.uk/plsp/applicant/
- Consent to share information. When registering, tick the consent to share information box. See our confidentiality policy.
- Provide us with evidence of disability.

How do I get a Yellow Slip?
To access the electronic yellow slip:
- Go to your PLSP page.
- Right click on the yellow slip image.
- Right click on the copy image.
- Paste image into a word document.
- Alternatively, the image can be dragged from the PLSP page straight into a Word document.
How do I set up my 121 study skills support?
To arrange your one to one specialist study skills and strategy support, please e-mail specialist121support@bangor.ac.uk.
I need to register with a GP
The University has contracted with one local practice to provide special services for students.
Currently this contract is held by Bodnant Medical Centre, Menai Avenue, Upper Bangor.

You can register with the surgery by completing a GMS1 form.
I want to be assessed for Dyslexia
To book an assessment for Dyslexia please contact the Diagnostic Assessment Service.
If you are unsure whether or not you require the full assessment we also offer a preliminary screening via email questionnaire.
Once this is complete, we will discuss with you the next steps for support which you may require.
The screening questionnaire is free of charge to current and prospective Bangor University students.

The Assessment Centre has created information to assist in Applying for DSA.
